The most important rule for the first 72 hours after bioidentical hormone replacement therapy is simple: protect the site. That means skipping vigorous workouts, heavy lifting, soaking (pool, hot tub, bath), and tight clothing that rubs or traps sweat over the area. I hear it all the time in our Desert Ridge office: people are pleasantly surprised their pellets were “not painful at all, ” and they want to keep recovery just as smooth.
The first 24–48 hours: keep it clean, dry, and low-impact
In Phoenix, heat is a real factor. Sweat plus friction is usually what stirs up irritation, and it can also mess with patch adhesion if you’re using transdermal BHRT hormone replacement. Here’s what I generally recommend right after therapy, with the timeline depending on your delivery method.
Quick timeline by method
- Injections: plan for 24–48 hours of lighter activity. Keep the area clean and avoid rubbing it.
- Patches/creams: avoid soaking and heavy sweating for 24–48 hours. Watch for peeling edges on patches, especially in the Phoenix heat.
- Subcutaneous pellets: be stricter for a full 72 hours. No heavy lifting, no high-impact workouts, and no soaking. Protect the insertion area from pressure and friction.
Gentle walking is usually fine unless I’ve told you otherwise. But skip the stuff that spikes pressure or pulls at the area: sprint intervals, heavy leg days, hot yoga, long hikes like Piestewa Peak, and anything that leaves you drenched.
And don’t “test it” by pressing around the site. Keep clothing loose. If you’re commuting around the Loop 101 near North Tatum Boulevard, bring an extra light layer so your seatbelt doesn’t rub directly on the area if it’s near your hip.
Days 3–7: when you can get back to the gym, golf, and pool plans
Most people feel ready to do more before the site is ready. That’s the trap. For pellets, I want you to treat the first 72 hours as non-negotiable. After that, you can usually start easing back in.
If you’ve been on hormone replacement therapy for a long time, like the clients who tell me we were able to “tweak a few things” so they felt more like themselves again, that momentum matters. Staying consistent with your dosing schedule and respecting recovery helps us read your response clearly.

For injections or patches, that return-to-activity window is often faster. Still, if you notice increasing redness, swelling, warmth, drainage, fever, or severe pain, don’t push through it. Pause workouts and reach out.
Site care that prevents bumps, irritation, and pellet problems
Good aftercare is boring on purpose. Keep the area clean and dry. Avoid rubbing, scratching, and tight waistbands that press on the spot. Don’t apply topical products directly over an insertion wound until it’s healed.
If you’re using a patch
- Apply to clean, dry skin. Lotion and body oil are common reasons patches peel.
- In the Phoenix heat, sweat can loosen adhesive. Try to avoid heavy sweating for 24–48 hours after a new patch goes on.
- Don’t “double up” without guidance. If a patch comes off, follow the instructions you were given and message us if you’re unsure what to do next.

When to reach out fast (and when to relax)
Mild soreness or a little tenderness near an injection or insertion site can be normal early on. What I don’t want you to ignore is a trend in the wrong direction.
- Reach out immediately for fever, severe pain, increasing redness, swelling, warmth, drainage, or any sign the pellet is working its way out.
- Reach out soon if you’re confused about dosing timing, patch issues, or you’re not sure what activity is safe for you.
- Keep going with your prescribed schedule. Don’t stop or change your hormone replacement therapy plan without talking with me.
